Our Care Managers are professionals with degrees in social work, nursing and related fields, and have many years experience in geriatrics and long term care.

• Assistance with eligibility for the Arizona Long Term Care System (ALTCS) and other long term care funding sources
• “Peace of Mind” visits  – visits to your loved one by a Care Manager as frequently as you choose, with telephone and written feedback on the visit
• Accompany your loved one to medical appointments, and communicate results of visit directly with family
• Coordinate services and advocate for your loved one when you can’t be there
